To schedule an appointment at any of our offices with one of the JWM neurologists, call 317-308-2800 (Anderson/Elwood locations: 765-298-4545) today. We're open Monday through Friday from 8:00 a.m. - 5:00 p.m. by appointment. If you are unable to keep your scheduled appointment, please try to notify us by telephone 24 hours in advance, or call as soon as you can. This will allow us to give your time slot to another patient.
We're committed to seeing patients as quickly as possible. If you need to be seen the same day you call, we will do everything possible to schedule you with one of our neurologists. And, we're available 24 hours a day for emergency situations. We also do our best to keep on schedule, but patient emergencies sometimes arise. We ask for your patience and understanding in these situations that may cause you to wait.
New patients are requested to complete patient registration forms. You can print your registration forms here. Your signature on these forms authorizes our staff to complete insurance forms for you.
Each time you visit our office, be sure to bring a list of all medications you are presently taking and the actual container with the label attached. You should also bring a list of observations of your condition. Consider logging this information into a health care logbook so you can keep notes about your problems and progress. This will provide your JWM neurologist important information.
If you are a new patient, you will need to complete some brief paperwork before your visit to JWM Neurology. Simply print our JWM Neurology New Patient Packet and complete the information.
If your appointment is more than one week away, please U.S. Mail this paperwork to the JWM Neurology office where your appointment will take place (see the “Our Locations” portion of our website for specific mailing addresses). If your appointment is less than one week away, please bring this completed paperwork with you to your appointment.
Please do NOT email these documents to us. Thank you.
