Scheduling Your Appointment

Thank you for choosing JWM Neurology. To schedule an appointment with any of our physicians, please call us:

Our offices are open Monday through Friday from 8:00 a.m. – 5:00 p.m. by appointment. We’re committed to scheduling patients in a timely manner. And, our physicians are available 24 hours a day for emergency situations. If you are unable to keep your scheduled appointment, please notify us by telephone no later than 24 hours in advance. This will allow us to give your time slot to another patient. If you are a new patient, please arrive 30 minutes ahead of your scheduled appointment time so that we can complete your registration. If you are an existing patient coming to JWM for a return visit, please arrive 15 minutes ahead of your scheduled appointment time.

Preparing For Your Appointment

Each time you visit our office for your appointment, please bring an up-to-date written list of all medications you are taking, including dosage and frequency. Be sure to include any over-the-counter medications, vitamins, herbal substances, supplements, etc. You should also bring a list of observations regarding your condition, including any symptoms you are having. Consider logging this information into a notebook so you can write notes about your symptoms and progress. This will be helpful information for your JWM neurologist during your appointment. Please make sure you have your current health insurance information available to bring to your appointment.

What to Bring to Your Appointment

A written list of current prescription and over-the-counter medications you are taking, including dosage and frequency (include vitamins, herbal substances, supplements, etc.)
Your health insurance card
Your driver’s license or other official form of identification
Physician referral forms if required by your insurance
Pertinent information about your medical and surgical history
Any recent imaging tests such as MRI, EEG, CT, PET CT results or appropriate records you may have
