Our offices are open Monday through Friday from 8:00 a.m. - 5:00 p.m. by appointment. We're committed to scheduling patients in a timely manner. And, our physicians are available 24 hours a day for emergency situations.
If you are unable to keep your scheduled appointment, please notify us by telephone no later than 24 hours in advance. This will allow us to give your time slot to another patient. If you are a new patient, please arrive 30 minutes ahead of your scheduled appointment time so that we can complete your registration. If you are an existing patient coming to JWM for a return visit, please arrive 15 minutes ahead your scheduled appointment time.
Each time you visit our office for your appointment, please bring an up-to-date written list of all medications you are taking, including dosage and frequency. Be sure to include any over-the-counter medications, vitamins, herbal substances, supplements, etc. You should also bring a list of observations regarding your condition, including any symptoms you are having. Consider logging this information into a notebook so you can write notes about your symptoms and progress. This will be helpful information for your JWM neurologist during your appointment. Please make sure you have your current health insurance information available to bring to your appointment.
If you are a new patient, you will need to complete some paperwork before your visit to JWM Neurology for your appointment. Simply print our JWM Neurology New Patient Packet, and complete the information.
If your appointment is more than one week away, please U.S. Mail this paperwork to the JWM Neurology office where your appointment will take place (see the Locations area on our website for specific mailing addresses). If your appointment is less than one week away, please bring this completed paperwork with you to your appointment.
